Smoking and Cellulite

You may have wondered if smoking and cellulite go hand in hand. Smoking is one of the most widely practiced bad habits in the United States. Cholecystectomy or Operation on the Gall Bladder ” Part Two

After the operation

Stitches and clips If the surgeon has used sub-cuticular clips then they do not need removal but if skin clips or stitches are employed then removal will be required at about a week after insertion. The small incisions are often controlled by using adhesive strips which gradually come off in whilst showering. Patients should be given clear advice on managing their wounds and stitches.

Recovering

Being discharged home The level of pain and difficulty patients have, their fitness levels and the presence or absence of an adult to assist them determines how quickly patients can return home. Typical discharge times are a day or two after the laparoscopic operation but if the operation has been an open one then it is usual for discharge to be a few days later. As a general principle people can go home when they feel able to.

Painkillers and pain The level of pain suffered by patients varies greatly as most suffer some pain symptoms in the first three or four days but others feel very little pain or discomfort. The typical area for pain is around the incision sites but some pain can be experienced in the shoulder region, likely due to the diaphragm becoming irritated during the operation. As carbon dioxide is absorbed into the bloodstream the discomfort will settle. It is advisable to use painkillers over the first days so that patients can sleep comfortably and start to become active again. The pain should resolve by five to ten days from the operation. A slower recovery after an open procedure is to be expected.

Showering and bathing The wound can be gently washed with water and soap in the shower or the bath after a couple of days. A see through dressing can be used by some surgeons and can be left on for a few days even when bathing and washing, although this will usually be instructed. Swimming is unwise until the wound is dry and sound which is likely to be about ten days from the procedure.

Getting about Getting up and walking about can be done as soon as the patient wishes and although there is no limit on this it is likely stiffness and some tiredness might limit the distances possible for about a week.

Driving a car Patients need to feel able and confident that they can control a car in an emergency situation before they drive, taking a few days to reach this point.

Sport, heavy lifting and work Return to work can be accomplished as soon as a patient feels comfortable enough to cope with the job demands. Home or part-time workers can usually resume work very soon after the event. Driving to work or spending a lot of the day on the feet delays work return for about two weeks. Full time work is usually achievable for most people after laparoscopic operation by ten days from the procedure but if they have had an open operation this will be a few days more.

Physical activities and sports can be resumed as soon as he patient’s discomfort allows but a graded return to these pursuits would be advised as fitness is regained. A month or more may be needed before resumption of contact or violent sports.

Common other side effects The wounds, either the open incision or the small incisions used to insert the laparoscope, usually show some bruising, hardness and swelling. The collection of blood and wound fluid under the incision is responsible for the swelling and hardness at first, contributed to also by stitch drawing together and lastly by formation of scarring.

What problems can occur after a cholecystectomy?

Haematoma The possibility of bleeding occurring has been mentioned already but at times there can be extensive bleeding if a small artery or vein under the wound bleeds for a while after the operation. This looks like a firm lump and usually settles down without treatment.

Bacterial infections It is not common to have a wound infection but it remains a risk. Antibiotic treatment may be needed if the wound should develop redness. Draining of any pus from the wound indicates that surgical exploration, release of infection and washout may be needed.

Surgery to the Gall Bladder or Cholecystectomy ” Part Three

Main bile duct injury The greatest risk during cholecystectomy is injury to the main bile duct. Once in every three hundred operations is the typical frequency of this problem. As it requires major surgery to fix this problem all surgeons are acutely aware of it and make every attempt to avoid it.

Other side effects in the abdomen There are other side effects from cholecystectomy such as a leaking of the bile but these are not usually apparent until after discharge of the patient. Patients should consult their general practitioners at once if they develop a yellow colour to the skin (jaundice) or find they are having steadily increasing abdominal pain.

Diarrhoea A small percentage of patients notice their bowels become looser after cholecystectomy. This is usually very minor but occasionally medication is needed to control this effect.

Deep vein thrombosis (DVT) DVT is a possible problem after cholecystectomy but is uncommon. If a patient is at particular risk (having had a thrombosis before or if they are taking the contraceptive pill) then they should tell the surgeon about this and special precautions will be taken to reduce the risk. Moving the legs and feet as soon as possible after the operation and walking about early all help to stop thrombosis occurring.

General anaesthetics ” the risks A general anaesthetic comes with some risks which are usually low but may be more likely in people with other medical diseases:

Common temporary side-effects (risk of 1 in 10 to 1 in 100) include bruising or pain in the area of injections, blurred vision and sickness (these can usually be treated and pass off quickly.

Infrequent complications (risk of 1 in 100 to 1 in 10,000) include temporary breathing difficulties, muscle pains, headaches, damage to teeth, lips and tongue, sore throat and temporary difficulty speaking.

Side effects which are very uncommon but very important with a risk of less than 1 in 10,000 could be long-lasting damage to blood vessels and nerves, failure of the liver or kidneys, injury to the eyes, brain, laryngeal or lung damage, serious allergic reactions and death. These latter side effects are very rare and depend on the other medical conditions the patient has.

The benefits of cholecystectomy via the laparoscope The benefits of laparoscopic cholecystectomy have meant that surgeons have changed almost completely from open operation. A speedier recovery time, lower infection risk, reduced post-operative pain and very limited scarring are the main reasons this operative technique is preferred. After 24 hours most patients are now mobile and able to go home, returning to work by seven days. Laparoscopic cholecystectomy is no longer a new operation and surgeons are skilled at doing this.

How to deal with problems A fever or infection discharging from a wound is an acute problem which sometimes presents and patients should initially contact their general practitioner for advice. Referral to the hospital surgeons may be necessary and the doctor will make the necessary calls should this be required. If local urgent medical consultation is not available from their general practitioner then attendance at the local Emergency Department should be sought.

PCS or Postcholecystectomy Syndrome

Post cholecystectomy syndrome (PCS) describes a group of symptoms which can present after cholecystectomy. The typical symptoms originally attributed to the gallbladder may continue after the operation or new symptoms can develop which would normally be thought to be due to gall bladder problems. This syndrome will also cover symptoms brought on by gallbladder removal itself. The cause of PCS is thought to be an alteration in the bile flow because the reservoir for bile, the gallbladder, has been lost. There may be an increased flow of bile into the upper digestive tract, adding to inflammation of the oesophagus and stomach. Secondly this may have an effect lower down in the digestive tract, causing diarrhoea and sharp abdominal pains.

Typical average prevalence of PCS is 10 to 15 percent of cases of cholecystectomy, with attentive communication and questioning required both to explain the potential difficulties pre-operatively and to elicit the somewhat subtle symptoms later. Overall opinion is that the more securely the diagnosis is made initially the less likely PCS is to occur.

Colles’ fractures, named after Abraham Colles who first described in 1814 the common fracture of the last inch of the radius and ulna near the wrist, is a very common consequence of a fall on the outstretched hand (FOOSH). Typical treatment is immobilisation in a plaster of Paris or similar material for five to six weeks to allow bony union, followed by a rehabilitation period of a month or more, a short period of which might involve a wrist brace for comfort during activity. Due to the functional importance of the hand, the period of immobilisation is kept to a minimum to prevent dysfunction of the hand and wrist.

Once the Plaster of Paris has been removed the physiotherapist will examine the wrist for appropriate healing by firmly palpating the area over the fracture, which should not show much more than mild tenderness. The hand should look a natural colour, have no tightness or swelling in the fingers and muscle wasting should not be severe. Movements of the wrist will be restricted in a few planes but should not be affected in all planes of motion, neither should there be severe pain on movement nor pain on all movements. If many problems are present the physiotherapist will take urgent steps to rehabilitate the patient.

Initial treatment is to instruct the patient in range of motion exercises to be performed every two hours. For many fractures this is all that is required as the movements are easily restored with a few days’ exercises, concentrating on the end ranges of movement. The shoulder and elbow are checked to make sure they are not limited as they may have been injured in the initial incident or kept very still by the patient whilst in plaster. The pronation and supination movements of forearm rotation are functionally very important, and the physiotherapist checks wrist extension and flexion and finger and thumb movements.

Patients often report that the wrist feels at risk after the plaster has been removed and this may be due to the early removal of the plaster to prevent functional loss from immobilisation. A futura brace, a fabric support stiffened with a metal piece under the wrist, is applied with Velcro straps to give support during normal activities of daily living. The brace should be taken off during rests or light activity and for regular performance of the exercises. Too much further immobilisation at this stage could be harmful so patients should understand the limited use of the splint for comfort during activity.

Joint mobilisations are used commonly by physiotherapists to improve joint ranges of motion if the exercises do not improve this alone. Physiotherapists perform accessory movements, so called mobilisation techniques, whereby they move the patient’s joint passively to re-establish the vital gliding and sliding movements. The midcarpal, radiocarpal (wrist) and lower radio-ulnar joints can be treated this way to increase the ranges, the physiotherapist fixing one part of the joint firmly as they move the other half. This can be done with gentle movements or much more strongly, pushing against the resistance of the stiff joint structures which are preventing full movement.

Strengthening the wrist occurs with a gradual increase in functional activities but joining a hand class can instruct the patient in practicing the large variety of small movements that the hand can perform and needs to strengthen for optimum hand function. Repetitive work at pieces of apparatus can strengthen and harden the hand to turning, twisting, pulling, grasping and fine work with the thumb and index finger. This can move on to work with weights or functional activities if the person needs to return to manual labour or another job requiring upper limb strength.

If the hand is very painful, swollen and restricted in motion then treatment may be urgently directed to preventing a pain syndrome developing, once the fracture has been reviewed by a doctor to make sure healing has progressed as it should. Hot and cold contrast bathing for the hand can be useful for the pain and swelling, with massage and sensory work to reduce the hypersensitivity which can be troublesome. Patients need to be very clear that they need to work hard through the pain in these cases to regain a normal hand.
